چکیده

Myocarditis is simply defined as an inflammatory condition mainly located in the heart muscle, although considerable uncertainties still persist as to its etiologic, pathological, and clinical subclassifications. In addition to the possibility of an aspecific flulike prodrome, possible presentations include different degrees of heart failure (ranging from cardiogenic shock to subtly progressive chronic heart failure), chest pain, conduction anomalies, bradyarrhythmias, and tachyarrhythmias (including sudden cardiac death). The clinical course is also variable, as a spontaneous recovery may occur after the acute phase in up to 50% of patients, whereas chronic myocarditis and dilated cardiomyopathy sustained by viral persistence and/or autoimmune selfperpetration represent a common evolution of the disease.

Feasibility of Rapid Linear-Endocardial and Epicardial Ventricular Ablation Using an Irrigated Multipolar Radiofrequency Ablation Catheter.

BACKGROUND A common strategy for ablation of scar-based ventricular tachycardia is delivering multiple lesions in a linear pattern. METHODS AND RESULTS We tested the efficacy of a novel linear irrigated multipolar ablation catheter capable of creating linear lesions with a single application. Healthy swine underwent endocardial and epicardial linear ablation using a novel linear irrigated abl...

Radiofrequency catheter ablation of idiopathic ventricular arrhythmias originating from intramural foci in the left ventricular outflow tract: efficacy of sequential versus simultaneous unipolar catheter ablation.

BACKGROUNDS Idiopathic ventricular arrhythmias (VAs) originating from the left ventricular outflow tract (LVOT) sometimes require catheter ablation from the endocardial and epicardial sides for their elimination, suggesting the presence of intramural VA foci. This study investigated the efficacy of sequential and simultaneous unipolar radiofrequency catheter ablation from the endocardial and ep...

Angiographic and electrophysiological substrates for ventricular tachycardia mapping through the coronary veins.

OBJECTIVE To study the value of epicardial mapping through the coronary venous system in patients with sustained ventricular tachycardia. DESIGN 20 consecutive patients with sustained ventricular tachycardia who were candidates for radiofrequency ablation. SETTING Electrophysiological laboratory. INTERVENTIONS Coronary venous angiography was performed with a catheter, which provided coron...

Epicardial-only block during endocardial mitral isthmus ablation facilitated by coronary sinus occlusion.

A 60-year-old man with obesity and severe left ventricular dysfunction and left atrial dilatation underwent catheter ablation of symptomatic, drug-refractory, persistent atrial fibrillation. After pulmonary vein isolation and electrogrambased ablation, atrial fibrillation was cardioverted successfully. Mitral isthmus ablation was undertaken during distal coronary sinus (CS) 1 to 2 pacing. CS bl...

Pathology after combined epicardial and endocardial ablation for ventricular tachycardia in a postmortem heart with hypertrophic cardiomyopathy

Introduction Ventricular arrhythmias in patients with nonischemic cardiomyopathy (NICM) often affect patients’ prognosis. Ventricular tachycardias (VTs) arising from the electrical scar area in hearts of NICM patients could become targets for radiofrequency catheter ablation (RFCA) therapy. However, the efficacy of RFCA using the endocardial approach is limited because of the deeper localizatio...

Combined epicardial and endocardial electroanatomic mapping in a porcine model of healed myocardial infarction.

BACKGROUND Substrate mapping of post-myocardial infarction ventricular tachycardia involves electroanatomic delineation of scarred tissue on the basis of electrogram characteristics during sinus rhythm. A percutaneous transthoracic technique was recently described that allows catheter mapping of the epicardial surface of the heart. This study sought to determine whether the epicardial extent of...
